What is Form 82-04 used for?
Form DS-82 lets eligible U.S. citizens and non-citizen nationals renew a passport book, card, or both by mail. You must meet every eligibility rule on the form, include a recent photo, mail your current passport, and pay the passport fee by check or money order.
Complete an accurate DS-82 ready to sign, attach a photo, and mail to the State Department with your current passport and fee.
If any eligibility answer is No, stop — you likely need Form DS-11 in person instead. Mailing an ineligible DS-82 wastes time and can delay travel.
