What is Form 51-AND used for?
Use FW-002 when you already have a California court fee-waiver order and need the court to waive additional costs, such as jury fees, witness interpreter fees, officer testimony fees, expert fees, or another listed cost.
Give the court a complete, clear request for additional waived costs tied to the existing fee-waiver order.
If your finances improved, the form says you must attach a new FW-001 request.
